Most "best fish oil" lists are the same fifteen Amazon links with the numbers moved around. We'd rather answer the questions people actually search for: what the label numbers mean, what's genuinely backed by evidence, and where the marketing runs ahead of the science. Some of it will sound less exciting than a typical buying guide. That's deliberate.
Omega-3 vs fish oil: what's actually the difference?
People use these terms interchangeably, and that's where a lot of the label confusion starts. Fish oil is the source, the oil extracted from the tissue of fatty fish. Omega-3 is the family of fatty acids inside it, and it's a bigger family than most labels let on. The two that matter most for human health are EPA and DHA, both found in fish oil. A third, ALA, comes from plant sources like flaxseed and walnuts, and the body converts it to EPA and DHA only inefficiently, a few percent at best.
So the real omega-3 vs fish oil difference is this: fish oil is one delivery method for omega-3, not the only one. Algae oil supplies EPA and DHA directly, without going through a fish at all, which is why it's the option for anyone vegan, vegetarian, or simply not keen on fish burps. A supplement can be "omega-3" without being fish oil. It can't be fish oil without containing omega-3.
Does the NHS actually recommend fish oil supplements?
Not in the way most product pages imply. NHS dietary guidance, through the Eatwell Guide, recommends at least two portions of fish a week, one of them oily, as a food source of long-chain omega-3. That's a recommendation about fish, not about supplements.
On the supplement side, the picture is more specific than most marketing suggests. NICE's current cardiovascular guideline (NG238) states plainly that omega-3 fatty acid compounds should not be offered to prevent cardiovascular disease, with one narrow exception: a specific prescription-only form used alongside statins for people with high triglycerides. Standard over-the-counter fish oil doesn't fall under that exception. We think that's worth saying directly rather than letting a vague "supports heart health" line do the work instead.
None of this means fish oil is pointless. It means the honest case for it is narrower and more specific than most sellers make it sound, which is exactly what the next few sections get into.
What a dietitian actually checks on a fish oil label
There's no shortage of products claiming to be dietitian approved fish oil UK shoppers can trust, usually without saying which dietitian, or what they actually checked. Rather than make that claim ourselves, here's what that kind of scrutiny actually looks like in practice.
A dietitian looks past the headline milligram figure to the combined EPA and DHA content, since that's the part with an evidence base, not the total oil weight. They check the form the fat is in (more on that below), whether the product has independent purity testing behind it rather than just a claim on the box, and whether the dose on the label matches what a specific goal actually requires rather than a generic "supports wellbeing" amount. That's a checklist you can run yourself, and it's more useful than a badge with no name attached to it.
1000mg vs 2000mg fish oil: what the front-of-pack number doesn't tell you
This is one of the most common points of confusion, and it's an easy one to clear up. The 1000mg vs 2000mg fish oil figure on the front of most bottles refers to total fish oil weight, not the amount of active EPA and DHA inside it. Fish oil itself is only ever partly omega-3; the rest is other fats and, in cheaper products, more filler than you'd expect.
A concentrated 1000mg softgel can easily contain more usable EPA and DHA than a standard-strength 2000mg one. The only way to compare two products properly is to look at the EPA plus DHA figure specifically, not the oil weight on the front. As a reference point: the authorised claim for normal heart function requires 250mg combined EPA and DHA a day; the claims for blood triglycerides and blood pressure require considerably more, 2g and 3g a day respectively, doses that go well beyond a standard daily softgel and are typically only reached deliberately, under guidance. Most people taking a single daily capsule are nowhere near those higher figures, regardless of what "2000mg" printed on the front might suggest.

Wild-caught vs farmed. Wild-caught oily fish generally have a more favourable, naturally occurring omega-3 profile, though farmed sources can be well managed too; this matters more for sustainability than for the fatty acid content itself.

Fish oil vs algae oil. Covered above, but worth repeating on a shopping list: algae oil is the only direct vegan source of EPA and DHA, rather than the ALA-only options most plant oils provide.

EPA-dominant vs DHA-dominant formulas. EPA carries more of the evidence around mood and inflammation-related research, DHA more around brain and eye structure; general-purpose products usually blend both rather than leaning heavily either way.

Triglyceride form vs ethyl ester form. Natural triglyceride-form fish oil is generally considered better absorbed than the cheaper ethyl ester form used in some budget products; the label doesn't always say which one you're getting, so it's worth checking the product's technical specification if it matters to you.

Total oil weight vs combined EPA and DHA. The point made above, worth checking every single time regardless of what the front of the pack emphasises.

Third-party purity testing. Certifications like IFOS (International Fish Oil Standards) test independently for heavy metals, PCBs and oxidation, rather than relying on the manufacturer's own claim.

Freshness and oxidation. Rancid fish oil smells and tastes worse and does less for you nutritionally; a genuinely fresh product shouldn't leave a strong fishy aftertaste, and many include natural vitamin E as an antioxidant to help preserve that freshness.

Sustainability certification. Marks like the Marine Stewardship Council or Friend of the Sea indicate the fishery was independently assessed, which matters if sourcing is part of your decision.

Liquid vs capsule vs mini softgel. Purely a matter of what you'll actually take consistently; smaller "easy swallow" softgels exist specifically for people who struggle with standard capsule size.

Prenatal-specific DHA formulas. Pregnancy and breastfeeding increase DHA needs specifically, which is why dedicated prenatal formulas exist rather than a standard general-purpose one; check with a GP or midwife on dose and source, since fish oil advice for pregnancy differs from general adult advice.

Products marketed for eye health. Often built around lutein and zeaxanthin alongside omega-3, which is a different evidence base to the fish oil itself; worth knowing the two ingredient groups aren't interchangeable even when they're sold together.

Higher therapeutic doses (2 to 3g/day). These sit above what a standard softgel provides and are generally something to discuss with a GP or dietitian rather than self-select, particularly if you're on blood-thinning medication, since normal dietary fish is considered safe alongside anticoagulants but higher-dose supplements warrant that conversation first.
What changes if you're looking for the best fish oil for adults over 50
There isn't one best fish oil for over 50 UK product, but the priorities do shift a little with age. Triglyceride management becomes more relevant for some people in this group, which is where the higher, guided doses mentioned above come in, under a GP's advice rather than self-directed. Joint comfort is a common reason people in this age bracket try fish oil, and the evidence for it is genuinely mixed, covered properly below. Eye health products combining omega-3 with lutein and zeaxanthin become more commonly searched for too, often alongside general concerns about vision changes with age.
The other point worth raising directly: this is also the age group most likely to be on statins, blood pressure medication or blood thinners. Normal dietary fish intake is considered safe alongside these, but higher-dose fish oil supplements can have a mild effect on bleeding time, so it's worth a quick conversation with a GP or pharmacist before adding a concentrated supplement on top of existing medication, rather than assuming a food supplement is automatically fine to combine with anything.
Omega-3 for dry eyes: what the evidence actually shows
This is one of the more commonly searched claims, and it's also one where we'd rather give you the honest, slightly less flattering answer. The largest and most rigorous trial on omega-3 for dry eyes to date, the DREAM study, followed 535 people with moderate to severe dry eye disease for a year, comparing 3g of daily omega-3 against a refined olive oil placebo. Both groups improved. Neither did notably better than the other, on symptoms or on clinical signs, and a follow-up extension study a year later confirmed the same result when supplementation was continued or withdrawn.
That doesn't mean omega-3 has no role in eye health generally. DHA does carry a separate, well-established claim for the maintenance of normal vision at a much lower daily amount, 250mg. But that's a different, more modest claim than "fixes dry eyes," and the largest trial specifically on dry eye disease didn't support the stronger version. If dry eyes are a persistent problem for you, that's worth raising with a GP or optometrist directly rather than treating a supplement as the fix.
Fish oil for inflammation: what's realistic to expect
Fish oil for inflammation is another claim that's more nuanced than the marketing usually lets on. EPA and DHA are converted in the body into compounds involved in resolving inflammation, which is real, well-described biology. Where it gets less clear-cut is turning that mechanism into a reliable, general "reduces inflammation" outcome you can expect from a daily softgel. There isn't a UK-authorised health claim to that general effect, and the condition-specific trial evidence, in rheumatoid arthritis, for example, tends to show modest improvements in specific measures like joint tenderness rather than a broad anti-inflammatory effect across the board.
If a joint or inflammatory condition is the reason you're considering fish oil, it's worth treating it as a genuinely uncertain, worth-trying option rather than an established treatment, and worth mentioning to whoever manages that condition rather than adding it on your own and hoping.
Where this leaves you
There's a real, specific case for fish oil: reaching your baseline omega-3 intake if you don't eat oily fish twice a week, and, at the higher end, supporting triglyceride management under proper guidance. There's a much weaker case for it as a general cure-all for hearts, joints, eyes and mood all at once, however the label reads. Check the EPA and DHA figure rather than the oil weight, check the form, and check whether the specific claim you're chasing actually has evidence behind it at the dose you're taking.
